Good morning.
I now have an answer for you: you should never store syrup in the fridge, even if it has been opened, because the sugar crystallizes in the fridge. That seems to have happened here. If, as in this case, the crystallization has already occurred, simply heat the syrup in a water bath for 30 minutes to a good 40 degrees to make it homogeneous again.
